- A few days before your appointment you will receive an email “Dr. Trevor Brooks Plastic Surgery Patient Portal Form Request”
- The email link will prompt you to access the free app on your i-phone or android device with a temporary password
- Log in
- Complete your medical history
- Complete your consent forms
- Securely communicate photos with our staff
Your forms are submitted for the nurses and our medical director, Dr. Trevor Brooks, to review before your treatment takes place.
